EARLY BIRD REGISTRATION FEES POSTMARKED ON OR BEFORE April 15, 2008: $150.00
REGISTRATION FEES POSTMARKED AFTER April 15, 2008: $175.00
Registration includes program, syllabus, continuing education credit, continental breakfast and luncheon.

Cancellation Policy:All monies will be refunded, less a $20.00 administrative fee, if cancellation is received in writing before April
15, 2008. After April 15, 2008, no refunds will be made, although you may transfer your registration to another person. Cancellation not
allowed on group registrations - see group discount policy.

GROUP DISCOUNTS AVAILABLE!

  • Groups of 5 or more: $130.00 per person.
  • Must be postmarked on or before April 15, 2008
  • Send all registration forms together with one form of payment
  • Cancellation is not allowed on group registrations, but you may substitute another person.
